The accredited Certificate IV courses is provided through a collaboration between TESOL Australia and Sydney Higher Education Institute (SHEI). Delivery and assessment of the course will be facilitated through SHEI, RTO Provider No. 45393

In which YEAR did you complete that school level?
A Unique Student Identifier (USI) is like a tax file number. It is unique to an individual person and stays with you for life. It is a reference number made up of numbers and letters that will be linked to the Australian National Vocational Education and Training (VET) Data Collection. A USI makes it easier for students to find and collate their VET achievements into a single authenticated transcript. It will also ensure that students' VET records are not lost. From 1 January 2015 it is mandatory for all Australian students or students studying in Australia to have a USI. If you do not have a USI, please go to usi.gov.au to create one.
Please note, if you are an international student studying offshore then you do not need to obtain a USI - this will mean that the results of the training will not be accessible through the Commonwealth and will not appear on any authenticated VET transcript prepared by the Registrar.
If you are an Australian student or you are studying in Australia then you must obtain and provide a USI.

You are able to claim a Credit Transfer for a unit if you have already been granted competence by an RTO in that unit. When claiming a Credit Transfer, you must present your original Certificate or Statement of Attainment showing the unit code and title. If the unit code and title do not match exactly, you may apply for RPL for that unit.

In order for you to gain entry into the SHEI Certificate IV course, you need to have a sound level of ability in written and spoken English. One of the below examples of evidence of English proficiency must be provided prior to enrollment in the course. The SHEI Admissions Team will then assess if you meet the course requirements based on the evidence provided.

Under the Data Provision Requirements 2012, SHEI is required to collect personal information about you and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER).
Your personal information (including the personal information contained on this enrolment form), may be used or disclosed by SHEI for statistical, administrative, regulatory and research purposes. SHEI may disclose your personal information for these purposes to:
- Commonwealth and State or Territory government departments and authorised agencies;
- and NCVER.
Personal information that has been disclosed to NCVER may be used or disclosed by NCVER for the following purposes:
- populating authenticated VET transcripts;
- facilitating statistics and research relating to education, including surveys and data linkage;
- pre-populating RTO student enrolment forms;
- understanding how the VET market operates, for policy, workforce planning and consumer information;
- and administering VET, including program administration, regulation, monitoring and evaluation.
You may receive a student survey which may be administered by a government department or NCVER employee, agent or third party contractor or other authorised agencies. Please note you may opt out of the survey at the time of being contacted.
NCVER will collect, hold, use and disclose your personal information in accordance with the Privacy Act 1988 (Cth), the National VET Data Policy and all NCVER policies and protocols (including those published on NCVER's website at www.ncver.edu.au).

Course commencement begins when Learning Portal course logins have been received.
On the day of commencement of the course there will be no refund available. A formal request can be made in writing for a refund stating your reasons for withdrawal. However, in most circumstances a future credit to complete the same course at a later date and time will be offered.
In this agreement, indemnity and waiver; TESOL Australia Trainers, means those persons employed and/or contracted by TESOL Australia for the purposes of providing training.
AGREEMENT, INDEMNITY & WAIVER COVERS THE FOLLOWING:
In the case of any in person or online training and/or practicum, I agree to abide by the rules and regulations of TESOL Australia as listed below and, follow instructions of their trainers relating to safety & health WHS and working with children. If I do not follow the WHS instructions given by TESOL Australia trainers I understand that, my enrolment may be withdrawn and the provision of training ceases and, I will not be entitled to a refund.
1. I agree not to hold TESOL Australia liable for any injury, loss or damages which may be personal, financial or otherwise that I may cause or which may result directly or indirectly as a result of my actions during or as a consequence of my training.
2. I hereby voluntarily waive any and all claims or actions, both present and future that may be made by my family, estate, personal representative, heirs, or assigns resulting directly or indirectly from training provided by TESOL Australia.
3. I state that all personal information I have supplied to TESOL Australia is true and correct. That all work; including assessments I submit to TESOL Australia, will be my own.
4. The use of: abuse, offensive language, racism, or derogatory remarks in any form, whether spoken or written whether to a TESOL Australia employee, trainer, student/s or affiliate school employee is strictly prohibited. Enrollment may be terminated without refund for violating these guidelines.
5. We reserve the right to disclose your information to appropriate services in cases where there is a threat of harm.
I have read and understood that this INDEMNITY AND WAIVER is intended to be as broad and inclusive as permitted by the law and agree that if any part is held invalid, the remaining parts of this INDEMNITY AND WAIVER will continue in full force and effect as intended. I further agree that the venue for any legal proceeding shall be the State of Queensland.
I understand the rights that I am waiving and that I am freely signing this INDEMNITY AND WAIVER. I have read and fully understand that by signing this agreement I am giving legal rights and remedies, which may be available to me.
